The ring of hair by the FTM's ear is perceived unconsciously as an earring.
Jaw strikes me as unnatural, and i suspect it has been digitally broadened at the bottom. And the whole lower half of the face has been imported from a man. It seems to have a different color and texture. Note how far the nose is from the upper lip.
That's my best guess on this one: an intact male's jaw pasted into a pic of an FTM. And the teeth messed with.

An extra glob of "flesh" has been tacked as an extension onto his left middle finger.
Note hand sign on with other hand. That hand is oddly blurred, too, suggesting the sign was adjusted later.
