Suspicious-looking. BUT in the vid as he talks, distinct creases appear and disappear that make it look like a tongue when it isn't.
People, at least some, naturally get that crease that makes it look like a tongue on the lip.
Furthermore i have noticed in other articles that some women leave the edges of their mouth without lipstick, which can create a similar illusion to some extent.

I really don't know about these last 2. Actually this guy's lip is probably just shiny here, and likely enhanced with filler.

I'd speculate 4 tongues here. The one on the post-op MTF ho right is thin though.
